FIELD: medical engineering; prosthetic dentistry. SUBSTANCE: device has casing having punch holder with punch and conic clip in carrier engageable with support over the spheric surfaces and also holder with die. The punch and the conic clip are provided with motion drives. Casing and die holder are united by means of threaded bayonet, and their drives are two manually rotatable levers engageable with corresponding ratchets through spring- loaded pawls. One of ratchets has thread engageable with the corresponding threaded portion of the punch holder and the other one has flat surface inclined with respect to device axis and engageable with end face surface of the carrier. Cylindric screens with ring- shaped bases are set coaxially with the ratchet of the punch to enable their rotation and fixation relative to the ratchet teeth.
