FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used in treatment of patients with ununited fractures and with false joints of long tubular bones, as well as in case of osteomyelites and oncologic bone pathology. For this purpose X-ray and computed tomographic examination of zone of bone distraction regenerate are carried out. Trocar is preliminary transcutaneously placed in projection of zone of distraction regenerate defect through opening. Release of zone of distraction regenerate defect from scar-altered connective tissue component by fan-like drilling is performed under intra-operative control of electronic-optic converter. After that, zone of defect of distraction regenerate is injectionally loosely filled with gel-like nanostructured composite implant through trocar opening. Implant contains enriched with platelets autoplasma, mixed in ratio 1:(1-2) with granules of complex alloplastic preparation on the basis of hydroxyapatite with 50-60 wt % of collagen. Enriched with platelets autoplasma is obtained by sampling 420-450 ml of blood 2-4 hours before operation with further double centrifugation, first at 2300 rev/min for 5 minutes with separation of erythrocyte mass from plasma. After that, plasma is centrifuged at 4000 rev/min for 5 minutes with further separation of supernatant liquid and obtaining 20-30 ml of enriched with platelets autoplasma. Remaining part of erythrocyte mass and plasma is returned intravenously by drop infusion into patient's blood stream during operative intervention or at early postoperative period. Gel-like nanostructured composite implant can additionally contain colloidal solution of nanoparticles of zero-valent metal silver Ag0, or gold Au0, or copper Cu0, or palladium Pd0, or platinum Pt0, also without admixtures of cations of said metals in amount 0.08-2.8 wt %. Size of introduced into implant metal nanoparticles constitutes from 2 nm to 40 nm. At the stage of preparation to implant application colloidal solution of nanoparticles of metals is additionally introduced into granules of complex alloplastic preparation with their further laying on the layer of enriched with platelets autoplasma in selected ratio.
EFFECT: method ensures efficient stimulation of distraction regenerate of tubular bones with delayed maturing due to stimulation of process of reparative ostogenesis in the zone of defect with simultaneous elimination of allergic reactions.
