SUBSTANCE: canals reaching an obturator membrane are formed according to the modified Manchester operation preceding a transection of cardinal ligaments under the vaginal mucosa from both sides of the uterine cervix by a blunt technique. After the ligaments are transected, and the uterine cervix is excised, transobturator hysterosuspension is performed. Guides with a narrow mesh implant are delivered in the transobturator direction at the level of the clitoris to the right and left femoral fold. The guides are removed. The implant is fixed with separate non-absorbable sutures to sutured stumps of the cardinal ligaments and to the uterine cervix. By drawing the free ends of the implant mesh, the uterine cervix is fixed in the small pelvis to compensate the genital prolapsed and adjoining tissues. The projecting excesses of the implant are removed. A mucous membrane of the anterior vaginal wall is restored. That is followed by a posterior colporrhaphia with levatoplasty according to the common technique.

EFFECT: reliable fastening and physiologically correct fixation of the uterine cervix, low number of intraoperative injuries, reduced load of organs and tissues, considerably reduced risk of complications and recurrences of the disease.
