METHOD FOR LAPAROSCOPIC RADICAL TREATMENT OF LOCALIZED PROSTATE CANCER USING PERCUTANEOUS ACCESS Russian patent published in 2021 - IPC A61B17/00 A61B17/94

Abstract RU 2756419 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to urology, oncology. Access to the abdominal cavity is provided. After retreating medially by 2 cm from the anterior-upper spine of the iliac crest, two ports are placed: on the left with a diameter of 10 mm, on the right 5 mm. Along the lateral edge of the anterior abdominal muscle, two 5 mm trocars are installed on both sides at the level of the navel. After filling the bladder with up to 200 ml of saline solution, a longitudinal incision is made in the projection of the bottom of the bladder. After visualization of the ureteral mouths, a circular incision is made with an indentation of 1 cm from the inner opening of the urethra, then a subfascial isolation of the prostate gland is performed to the level of the apex using the Thunderbeat device, cutting off the seminal vesicles. Next, the prostate is cut off from the urethra and immersed in a container. Hemostatic sutures are applied to the prostate bed using a V-Loc thread. A profiled 18Ch Foley urethral catheter is installed. The balloon of the catheter is inflated by 15 ml in the bed of the prostate gland. The bladder wall is sutured with a continuous suture using a V-Loc thread. 150 ml of saline solution is injected into the bladder, tightness is checked. Trocars are being removed. The prostate gland is evacuated using an endoscopic container. The wounds are sutured in layers.

EFFECT: method makes it possible to reduce the risk of intraoperative bleeding associated with the absence of the need for crossing and stitching of the dorsal venous complex, to reduce the negative impact of this factor on the course and results of the operation; reduce the risk of surgical damage to tissues and neurovascular elements, prevent intra- and postoperative complications associated with this circumstance; improve the quality of life of the patient in the postoperative period.
