METHOD FOR DETERMINING THE POSITION OF THE CATHETER DURING CATHETERIZATION OF THE RETROBULBAR SPACE IN CHILDREN Russian patent published in 2022 - IPC A61F9/07 A61B1/55 A61B3/12

Abstract RU 2785031 C1

FIELD: ophthalmology.

SUBSTANCE: invention can be used in the conservative treatment of intraocular pathology by means of catheterization of the sub-tenon space, mainly in children. To do this, a conjunctival incision 10 mm long is performed in the lower outer part of the eye with the formation of a sub-tenon tunnel. Then through the lower lip of the conjunctival incision the skin is punctured in the outer part of the lower eyelid with a 18G needle. Then a guidewire is inserted into the needle cavity, the needle is removed, then a catheter is inserted along the guidewire and advanced into the formed sub-tenon tunnel with subsequent removal of the guidewire. Further, a light pipe is inserted into the catheter cavity for the entire length of the catheter, an examination of the fundus is performed using a retinal camera or ophthalmoscopy to determine the position of the projection of the catheter’s end. Moreover, a glow from the light pipe on the fundus near the optic nerve head indicates that the catheter is located as close as possible to the sclera at the optic nerve. In the absence of luminescence the catheter together with the previously inserted light pipe, is advanced forward in the direction of the optic nerve until luminescence appears. Then the light pipe is removed, a continuous suture is applied to the conjunctiva, the position of the catheter is fixed with a patch, the end of the catheter is closed with a rubber stopper and the drug is administered.

EFFECT: invention makes it possible to perform conservative treatment in pediatric patients in full by delivering drugs as close to the sclera as possible near the optic nerve, as well as maintaining the maximum therapeutic effect without psychologically traumatizing the child.
