FIELD: medicine; traumatology and orthopaedics.
SUBSTANCE: invention can be used for osteosynthesis for fractures of the distal bones of the leg. A transverse direct surgical approach is performed with dissection of the skin and subcutaneous tissue. Penetration between the superior and inferior retinaculum of the extensor tendons is performed. Two windows are formed by blunt dissection, the first of which is located between the long extensor of the first toe and the long extensor of the toes, and the second is lateral to the long extensor of the toes and an anterolateral plate with angular stability of screws is installed. After installation of the anterolateral plate with angular stability of the screws, a longitudinal medial distal skin and soft tissue incision 1–2 cm long is made, starting proximal to the apex of the medial malleolus. A low-profile plate, previously moulded to the shape of the bone, with a length sufficient to cover the fracture zone, is passed epiperiosteally from the bottom up until its distal end reaches the lower edge of the incision, after which a vertical medial proximal skin incision is made at the level of the proximal end of the low-profile plate and the low-profile plate is fixed with screws through both approaches distal and proximal to the fracture zone.
EFFECT: method ensures stability of fixation, reduces the rate of failure of osteosynthesis and postoperative complications associated with the healing of postoperative wounds due to the fact that the approach to the fracture zone is carried out between the upper and lower retinaculum of the extensor tendons without damaging them.
