SUBSTANCE: the present innovation deals with diseases and damages of extrahepatic biliary ducts. One should mobilize mesenteric edge of small intestine and perform complete mobilization of inferior horizontal duodenal branch which should be completely withdrawn from under mesentery into dextraposition against the upper mesenteric vessels to apply additional 2-4 sutures by paraductal tissues that enables to put indications and perform plasty of extrahepatic biliary ducts with "end-to-end" anastomosis without tension along the line of anastomosis' sutures that prevents the possibility for repeated development of strictures.
