METHOD FOR PLASTY OF PERFORATED GASTRODUODENAL ULCER DUE TO TUNNELIZATION Russian patent published in 2005 - IPC

FIELD: medicine, abdominal surgery.

SUBSTANCE: on should perform incisions of serosa of 2-3 cm length to conduct tunnelization up to perforated ulcer. Moreover, one should make 2 incisions being parallel to circular fibers, then it is necessary to cut a fragment upon a pedicle out of seroso-muscular membrane of anterior gastric wall to apply it through the tunnel and place it upon the ulcer with serosa. Then one should fix the fragment to gastric and duodenal seroso-muscular membrane along the edges of serous incisions and along the edge of perforation. Defect of gastric wall should be covered with the strand of greater omentum. In peculiar cases, incisions should be performed 1-1.5 and 2-2.5 cm above and below the ulcer. Fragment's pedicle should be developed being 1-1.5 cm above the upper serosal incision. The innovation enables to improve circulation in gastroduodenal area in case of large perforated ulcers.

EFFECT: higher efficiency of plasty.
