METHOD FOR PLASTY OF PERFORATED GASTRODUODENAL ULCER WITH SEROSO-MUSCULAR FLAP DUE TO TUNNELIZATION AT DISSECTING ULCEROUS DEFECT Russian patent published in 2007 - IPC A61B17/00 

FIELD: medicine, urgent abdominal surgery.

SUBSTANCE: one should dissect ulcerous defect being 1 cm against perforated opening, wound's edges should be correlated in transverse direction due to traction by seroso-muscular flap and in such position the flap should be fixed. Moreover, the above-mentioned flap should be cut out of seroso-muscular membrane of anterior gastric wall to apply it through the tunnel. Tunnelization should be fulfilled through serosal incisions being parallel to circular fibers. On applying through the tunnel the flap should be applied onto the wound with serosa. The flap should be sutured with duodenal wall along the perimeter by not capturing mucous and submucous membranes. Then one should cover the defect at the site of the cut out seroso-muscular flap due to suturing up the strand of greater omentum. The innovation enables to apply the above-mentioned technique in combination of perforation and hemorrhage, provides shortened terms for wound healing, increases reliability and hermetic sealing of sutures, excludes deformation of organ's wall and disorders of gastric evacuatory function.

EFFECT: higher efficiency of plasty.
