FIELD: medicine, surgery.

SUBSTANCE: the present innovation deals with treating invasive cancer of the bladder. One should resect intestinal segment, reconstruct intestinal continuity, the bladder should be formed out of ileal segment, bladder's collum, the segment of urethra and artificial sphincter should be formed out of the second resected ileal segment of 10 cm length. For this purpose it is necessary to fulfill deserozation in transverse direction at segment's ends being 1.5 cm against the crossing line. The part of free mucosa should be inverted and sutured up to deserozation line. At 1.0 cm against deserozation line it is necessary to develop two rows of longitudinal incisions of serous and muscular layers, the length of each incision being 2 cm, the distance between the rows ranged 1.0-1.5 cm. It is necessary to bring together distal and proximal ends of every incision and suture in the walls of incisions in transverse direction. Moreover, one should fulfill reconfiguration of transplant's walls along narrowing the internal lumen up to 0.5 cm. Proximal intestinal end should be anastomosed with saved urethral fragment. The suggested innovation enables to develop the mechanism for retaining and regulated excretion of urine at low urethral resection.
