FIELD: medicine, thoracic surgery, phthisiology.
SUBSTANCE: the present innovation deals with surgical treatment of destructive tubercular forms along with bacterial excretion. It is necessary to fulfill intercostals thoracotomy followed by extrapleural separation of the top of affected lung to form extrapleural cavity. Then comes forced blowing of operated lung to detect possible lesions in parietal pleura. In case of confirmed defect, one should introduce an irrigator into pleural cavity and connect it with the system of active aspiration for complete and immediate removal of air, as for the defect it should be sutured in till hermetic sealing. Then one should form a soft-tissue fragment as a muscular one out of the broadest muscle of the back. Due to mobilizing the fragment, one should maximally keep long lateral and posterior sternal nerves and, also, muscular bundles in area of its fixation to brachial bone and auxiliary area. The fragment should be shifted into developed extrapleural cavity through thoracotomic foramen to fix it towards the cupola and thoracic costal surfaces without tension by excluding the prolapse and pressure upon the affected lung. The innovation enables to liquidate degradation cavity completely and radically by excluding repeated surgical interferences due to the suggested technique on fragment's development out of the broadest muscle of the back that clinically doesn't considerably affect respiratory biomechanics and has no unfavorable effects in case of applying other variants of extrapleural cavity filling.
