FIELD: medicine.
SUBSTANCE: invention relates to the field of medicine, namely to the thoracic surgery and can be used after resection of the upper parts of lung. The method is that a lateral thoracotomy is carried out from skin incision in the fifth intercostal space without resection of ribs, the lung is taken from pleural adhesions, avoiding damage of the parietal pleura and intrathoracic fascia in the cupula projection. Resection of the lung is performed, after inflation of the resected lung the area and localisation of extrapleural pneumolysis is determined. Skin and subcutaneous flap is dissected from the upper edge of the wound, for which the skin and subcutaneous tissue from the upper edge of the wound is sharply separated from the serratus cranially to the level of the third intercostal space, where after abstraction of the flap up serratus are defoliated along the fibers lengthwise 4 cm. Then portions of intercostal muscles are dissected to the intrathoracic fascia lengthwise 4 cm. In the third intercostal space the trocar is set through which the thoracoscope is inserted with optics and connected camcorder, the pleura and intrathoracic fascia are detached in extrafascial layer from the inner surface of the chest within the planned amount without damaging its integrity under the dual visual control: through a standard thoracotomy from inside of the pleural cavity and through videothoracoscope inserted from the outside to discussion of the third intercostal space. After hemostasis under control of videothoracoscopy microirrigator is inserted into extrapleural cavity through a separate puncture in the third intercostal space for subsequent evacuation of exudate and administration of antibiotics. This cavity is filled with filling material - a structured collagen, the serratus is hermetically sutured, the pleural cavity is drained and sutured.
EFFECT: use of this invention reduces the risk of postoperative complications, and enables to create a collapse of lung of a greater degree.
