METHOD OF SURGICAL TREATMENT OF SUBMANDIBULAR AND PTERYGOMANDIBULAR DIFFUSE PHLEGMONS Russian patent published in 2009 - IPC A61B17/24 

FIELD: medicine; dentistry.

SUBSTANCE: skin is dissected: one incision is made in submandibular region in parallel to mandible edge, and the second one takes retromandibular region between leading edge of bowing muscle and trailing edge of mandible ramus anteriad to back edge of submandibular gland at the distance 3.0 cm upwards and 1.5-2.0 cm downwards from a conditional point of mandibular angle on superior nuchal fold. Posteroinferior parotid gland along with a capsule is cut off from leading edge of bowing muscle and from submandibular gland. Posteroinferior parotid gland is displaced up anteriad. Trailing edge of mandible ramus is uncovered. Tendons of medial pterygoid muscle are dissected on internal trailing edge of mandible ramus in length 10-15 mm. Medial pterygoid muscle is displaced inward. Cellular space between internal surface of mandible ramus and external surface of medial pterygoid muscle is splitted. A surgical clamp is applied between external surface of medial pterygoid muscle and internal surface of mandible ramus to a wound in submandibular region. The incisions are jointed. It is followed with antiseptic management and installation of a slotted rubber tube.

EFFECT: improved quality of drainage, reduced risk of injuring marginal branch of facial nerve.
