SUBSTANCE: invention concerns medicine, namely to traumatology and orthopedics. It involves posterior approach by linear longitudinal layered dissection of skin, subcutaneous fat, shin fascia for a distance of 20 cm in a posterior medial projection of shin. That ensures appearing in an interfascial compartment between medial head of gastrocnemius muscle, soleus muscle, Achilles tendon, posterior neurovascular bundle of shin, tendons of posterior tibial muscle, long flexor muscle of toes. An autograft is prepared of interfascial fat and connecting tissue, with a base on a fascia of medial head of gastrocnemius muscles, soleus muscle, Achilles tendon. The graft is cut off along the longitudinal axis of tibia at 15-18 cm from fascial bed in the width up to 2.0 cm, crosswise transected, in a distal direction closer to calcaneal tuber. The autograft is moved to a bone cavity, through "a tunnel" laid subcutaneously with a tool by a blunt technique. It is fixed in the bone cavity transosseously with a Z-suturing. Fascial defect of gastrocnemius muscle, soleus muscle, Achilles tendon is sutured.

EFFECT: method provides complete treatment of osteomyelitis, reduced postoperative complications, relieved pain syndrome and reduced treatment time.
