SUBSTANCE: invention relates to medicine, namely to surgery. Method of selection of surgical treatment in case of acute destructive cholecystitis includes estimation of severity of physical condition of patient with acute cholecystitis by ASA classification. If I-II class of severity of patient's condition is stated, laparoscopic cholecystectomy is selected. In case of negative result of gallbladder isolation within 30 min, method is changed to cholecystectomy from miniaccess. In case of detecting massive bleeding, presence of cholecysto-duodenal fistula, method is changed to cholecystectomy by laparoscopic access. If III class of severity of patient's condition is stated, cholecystectomy from miniaccess is selected. In case of detecting massive bleeding, presence of cholecysto-duodenal fistula, inflammatory infiltrate, method is changed to cholecystectomy by laparoscopic access. Reduction of operation time makes it possible to reduce time of anesthesia and toxic effect of anesthetic medications.

EFFECT: reduction of method trauma makes it possible to reduce treatment terms nd reduce risk of post-operation hernia development.
