FIELD: medicine.

SUBSTANCE: invention relates to field of medicine and can be applied in cardiosurgery. Standard pre-operation preparation and instrumental examination of cardio-vascular system are carried out. Dissection of sternum, anterior surface of pericardium in the place of connection of right and left mediastinal parts of mediastinal pleura is carried out to perform operation on heart. As material for isolation of heart from posterior surface of sternum in anterior mediastinum in conditions of pericardium deficit, patient's mediastinal pleura is used. Mediastinal pleura separated from left mediastinal part of pericardium is from two sides partly separated from sterno-pericardial ligament at the top, and at the bottom along phrenic nerve without traumatising it, to the middle of long heart axis. Pleura is separated in such a way that it preserves its integrity and blood supply with diaphragmatic part of mediastinal pleura from one side. After that, heart and vessels are covered with separated flap and sutures are applied successively in direction from the edge of dissected sterno-pericardial ligament with right mediastinal part of pericardium and downward to diaphragm.

EFFECT: method makes it possible to completely isolate heart, large vessels and bypasses from extrapericardial process of commissure formation, which contributes to reduction of risk of re-operations, reduction of resternotomy trauma and following cardiolysis threatening with damage to different structures in thoracic cage.
