FIELD: medicine.

SUBSTANCE: during the first surgery, pericardium anterolateral departments are separated from the left mediastinal pleura, at distance of 1.5-2 cm from the vagus nerve. This dissection extends from the upper sterno-pericardial ligament to the diaphragm. Then the pericardium is cut along the heart blunt edge projection on the pericardium downward to the diaphragm and then laterally from left to right along the border of the front and diaphragmatic parts of the pericardium. A flap of pericardial anterolateral departments is formed. After completion of the main phase of surgery, prior to sternum osteosynthesis, the formed flap is fixed by individual interrupted sutures to the inner surface of the left sternum edge, covering the front surface of the heart. During the second surgery, after dissection of the soft tissues and the xiphoid process, under visual supervision, the pericardium is separated from the sternum in the bottom right corner of the wound, and reexploration is performed using a Gigli saw or a sternal saw.

EFFECT: method allows reliable prevention of heart injury during reexploration due to its reliable isolation at the first operation using its own pericardium, which provides natural high strength, while avoiding complications caused by heart compression and displacement of heart-insulating material.
