FIELD: medicine.

SUBSTANCE: defect is replaced with the skin-muscle and periosteal flaps on the leg. The base of the periosteal flap is located vertically along the edge of the outer wall of the orbit or up to the anterior border of the malar bone tubercle, so that the horizontal line that crosses it is in the range from the middle of the pupil to 3 mm higher. The sponge flap is cut out in the form of an arcuate strip with an angle of 85-120 between its horizontal and vertical parts° in the direction downwards along the anterior-lateral surface of the frontal process of the malar bone. At that, a tarsal plate or tarso-conjunctival graft or a mucoperiodial graft is fixed to the periosteal flap. If there is a thin membrane, the second arcuate flap is cut out of the same base upwards. The angle between the strips is 20-90°, at that, two strip form a duplication. The vertical size of the periosteal flap base is 5-14 mm, flaps length is 4-20 mm.

EFFECT: method provides re-establishment of the lower eyelid frame structure at its extensive defects, which provides adequate support to the lower eyelid and its fixation to the bone and medial canthus, reliable strengthening of the external canthus, that allows to maintain good adaptation at the later stages after surgery and lessen the effect of eyelid sagging, and, consequently, leads to better functional and cosmetic results of reconstruction.
