FIELD: medicine.
SUBSTANCE: percutaneous puncture of the femoral artery is performed. Then, serial catheterization of the femoral artery is performed by catheters of increasing diametre, leading them in the retrograde direction. In this case, a sheath with a diametre d1 is inserted through the puncture needle, the needle is removed, and a dilator with an outer diametre D1 is inserted through the sheath. After removing the dilator, a catheter with an outer diametre corresponding to D1 is inserted into the femoral artery lumen through the sheath. The sheath is removed, and a sheath with a diametre d2 larger than d1 is inserted into the femoral artery lumen through the catheter. The catheter is removed, a dilator with an outer diametre D2 larger than D1 is inserted through the sheath. After removing the dilator, a catheter with an outer diametre corresponding to D2 is inserted into the femoral artery lumen through the sheath. The sheath is removed, and a sheath with an outer diametre d3 larger than d2 is inserted into the femoral artery lumen through the catheter. The catheter is removed, a dilator with an outer diametre D3 larger than D2 is inserted through the sheath. After removing the dilator, a catheter with an outer diametre corresponding to D3 is inserted into the femoral artery lumen by the sheath; the sheath is removed. Further, a sheath is inserted through the installed catheter to install a cannula, through which one or two dilators are installed alternately to lead the cannula. After they are removed, the cannula is inserted into the lumen of the femoral artery in the retrograde direction and attached to the arterial ECMO line. Intervention is carried out simultaneously, in one stage.
EFFECT: adequate, non-invasive approach to the femoral artery during its peripheral cannulation for carrying out veno-arterial extracorporeal membrane oxygenation.
