FIELD: printing industry.

SUBSTANCE: printable form for identifying a patient having at least one blank tear-off bracelet containing a waterproof printable nylon tufted faceplate and a backing. The face layer is stamped with a stamp to form at least one unfilled tear-off bracelet having a first end and a second end; the nylon tuft layer being 4-6 mils (0.1016-0.1524 mm) in thickness and has a coating on the upper surface and on the lower surface. The coating on the upper surface of the face layer is configured to receive printable, mutable patient data. The bracelet is waterproof, soft for the patient's hand and is configured to remain attached to the patient for a number of days without loss of the printed patient data being changed. The face layer is adhered to the substrate with a contact adhesive applied to at least a portion of the lower surface of the face layer. The contact adhesive is applied to the face layer around the periphery of at least one bracelet, leaving a portion of said bracelet between said first and second ends not adhered to the substrate. The means for securing the bracelet to the patient's wrist are located next to the first end and the second end of the bracelet. Each of the mentioned first end and a second end of each of the bracelets has means for indicating the opening.

EFFECT: bracelet provides a reusable use, sufficient water resistance and can remain attached to the patient for several days without loss of the printed patient data being changed.
