SUBSTANCE: invention relates to surgery and can be used for radical duodenoplasty by a continuous two-level seam in a giant circular perforated ulcer of the duodenal bulb. Anterior wall of the duodenal bulb is transversely dissected through the center of the perforation, ulcerative defect of the anterior wall of the bulb of the duodenum is excised under visual control from the mucosa, omentoplasty is performed with a strand of gastrocolic omentum on the vascular pedicle, fixed by two superficial nodular seams in the area of the hepatoduodenal ligament. When creating duodenoduodenoanastomosis, a two-level continuous seam is used to form nodes – "fixators".
EFFECT: method allows reducing the risk of edge damage to the pancreas, accelerating the processes of reparative regeneration in the area of anastomosis, even distribution of the load on the cross-linked tissues.
