METHOD OF DISSECTION OF UNSUSTAINABLE PERFORATING VEINS IN SEVERE FORMS OF CHRONIC VENOUS INSUFFICIENCY IN ELDERLY PATIENTS WITH PRONOUNCED CONCOMITANT PATHOLOGY Russian patent published in 2018 - IPC A61B17/00

Abstract RU 2666402 C2

FIELD: medicine.

SUBSTANCE: group of inventions refers to medicine, namely, to vascular surgery. Open the skin and subcutaneous tissue at the site of operation with a cut length of 10–12 mm. Isolate the perforating bundle to its own fascia of the lower leg or thigh. Allocate a perforating vein, for this purpose make a cut up to 3 mm in length between the perforating vein and the remaining neurovascular elements. Further, mobilize a perforating vein with the aid of the claimed device, by conducting its working part deep into the superficial fascia and back out to the mouth of the perforating vein. Further, tie the isolated vein at a distance of 2–3 mm from the fascia with a 6/0 thread and transected. Then, transect the proximal segment of the vein at a distance of 1–2 mm from its surface mouth. After that, adapt the edges of the postoperative wound by applying 1–2 nodal skin seams with a 6/0 thread. Similarly, treat all targeted unsustainable perforants. Method is carried out by means of a device comprising a handle and a working part, made in the form of a hook with a radius of curvature of 4 mm, the end of which is 10 mm long bent at an angle of 120°relative to the longitudinal axis of the device.

EFFECT: group of inventions allows to reduce the probability of development of complications, to reduce the traumatism of the operation, to shorten the healing time of the postoperative wound.
