METHOD FOR ELIMINATING SOFT TISSUES PTOSIS OF THE MIDDLE AND LOWER FACE AREA, DISPLACEMENT OF LOWER LIP, LOWERING ANGLE OF MOUTH AND FORMATION OF NASOLABIAL FOLD IN CASE OF PARESIS AND/OR MIMIC MUSCLES PALSY Russian patent published in 2019 - IPC A61B17/00

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to maxillofacial and plastic surgery. Incision is made in the skin and subcutaneous fat in the prootic area from the base of the ear curl stem. Form the tunnel in the subcutaneous fat layer. Tighten soft tissue using non-absorbable surgical suture. In this case, the above incision in the prootic area is carried out along the trestle, then, bordering the earlobe 0.5–1.5 cm along the back ear crease. Mobilize skin-fat flap, then in the area of the alleged nasolabial folds perform two incisions in the skin and subcutaneous fat 0.3–0.5 cm long, the upper incision, retreating 0.5–0.6 cm from the base of the wing of the nose, lower – above the angle of the mouth by 0.5–0.6 cm. Between them form a tunnel in the subcutaneous fat layer, repeating the contour of the nasolabial fold of the contralateral side. Nonabsorbable surgical suture is conducted in the subcutaneous fat layer from the incision in the pre-anterior region to the upper incision in the nasolabial fold area. Further, through the formed tunnel to the lower incision, longitudinally passing through the muscles that form the nasolabial fold, the nonabsorbable surgical suture is led out through the subcutaneous fat layer into the prootic area. Eliminate ptosis of the soft tissues of the midface and form the nasolabial fold, tightening the ends of the non-absorbable surgical suture and fixing them subperiosteally to the zygomatic arch. Then two incisions are made in the skin and subcutaneous fat 0.3–0.5 cm long in the area of the angle of the mouth and in the midline of the lower lip, retreating 0.2–0.3 cm from the red border of the lower lip. Between the incisions form a tunnel in the subcutaneous fat layer, repeating the contour of the lower lip. From the prootic area in the subcutaneous fat layer, a non-absorbable surgical suture is conducted to the corner of the mouth, then through the formed tunnel through the muscle, which lowers the angle, and the circular muscle of the mouth. Then non-absorbable surgical thread is removed through an incision in the midline of the lower lip, fixed to the circular muscle of the mouth, rotated and passed through the tunnel and the subcutaneous fat layer in the prootic area. Eliminate ptosis of the soft tissues of the lower face area, displacement of the lower lip and lowering the angle of the mouth, tightening the ends of the non-absorbable surgical suture and fixing them to the fascia of the parotid salivary gland.

EFFECT: method allows to increase the functional and aesthetic effect of correction of ptosis of soft tissues of the middle and lower zones of the face.
