FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics and can be used in presence of considerable defects of a distal part of femur under condition of intact bones. Prepared allograft from the shin bone is put on the distal end of the resected femoral bone, then an intramedullary nail is retrograde introduced into the intramedullary canal of the femur through an allograft. Bone bed is formed on an articular surface of a proximal epiphysis of tibia with an opening penetrating into its intramedullary canal. Distal narrow end of the bone allograft is inserted into the prepared bed, an antegrade rod is inserted through the formed opening into a medullary canal of the tibia. Two bone autografts from the fibular bone are placed along the allograft, and then, through the installed fragments of the fibular bone and the epiclisis of the shinbone in the direction of the rod, at least two holes are made. Proximal ends of the grafts from the fibular bone are tightly applied to the allograft, and in the areas of their application, proportional grooves are made to the femoral bone, in which the above autografts are tightly implanted. Fixed ends of said autografts and femur are then perforated. They are fixed with wire cerclages, after which the detached muscles are tightly closed around the installed bone grafts.
EFFECT: invention provides one-stage recovery of length and support ability of lower limb in patients with creation of favorable conditions for adequate arthrodesis of involved knee joint due to replacement of defect of distal femur by combined allo- and autogenous bone plasty with fixation of connected parts of lower extremity and bone grafts by intramedullary nail and wire cerclages.
