METHOD OF SURGICAL TREATMENT OF EXTENSIVE DUODENAL INJURIES WITH SEPARATION OF MAJOR DUODENAL PAPILLA FROM MEDIAL WALL OF DUODENUM WITHOUT DAMAGING WIRSUNG'S DUCT AND CHOLEDOCH IN STOMACH INJURIES Russian patent published in 2020 - IPC A61B17/00

Abstract RU 2718289 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgical gastroenterology. Duodenum is mobilized according to Kocher. Further, duodenum is disconnected and removed to ligament of Treitz, leaving a strip of duodenum medial wall on the head of the pancreas. From the left strip, the mucosa is removed without affecting the major duodenal papilla (MDP). Thereafter, an abdominal end of the jejunum is transected to form a jejunal Roux loop that is brought to the left stripe of the duodenum medial wall. Further, MDP is sutured into the wall of the antimesenteric jejunal loop edge, receding below from the resected end of intestine, having previously inserted the stent into the Wirsung's duct. Then choledochojejunoanastomosis is formed "side-by-side", below which choledochosis is ligatured. Jejunal loop is fixed to the left wall of duodenum with 2 rows of interrupted sutures. Gastroenteroanastomosis and jejunoanastomosis are formed.

EFFECT: method enables reducing the risk of duodenum suture inconsistency by removing bile and gastric contents from the duodenum lesion and restoring passage of pancreatic juice.
