FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to oncology, abdominal surgery, and can be used for surgical treatment of patients with a tumor lesion of the duodenum (duodenum) that does not extend to the papilla of Vater and the peripapillary region of the duodenum. A duodenectomy is performed with preservation of the peripapillary flap of the duodenal wall with both duodenal papillae. The additional pancreatic duct of Santorini is preserved, as well as the anterior and posterior arches of the pancreatoduodenal vessels, the branches of which supply blood to the preserved flap. At the next stage, the initial section of the jejunum is moved to the right from the superior mesenteric vessels and a duodenojejunostomy is formed by suturing the preserved flap of the duodenal wall into a longitudinal incision on the antimesenteric edge of the jejunum.

EFFECT: invention is the method for choice in patients with non-epithelial, with the exception of lymphomas, neuroendocrine tumors and secondary tumor invasion of the duodenum from the outside; reduces the risk of duodenojejunostomy failure and pancreatogenic complications; nullifies the risk of developing cholangitis, exocrine and endocrine disorders of the pancreas in the long-term postoperative period due to the preservation of both duodenal papillae and the integrity of the pancreas.
