SUBSTANCE: invention relates to the area of medicine, namely to otorhinolaryngology. The tympanic flap is detached from the lower third of the malleus handle and the umbo area. Medially to the remnants of the tympanic membrane, a fascial transplant is placed and inserted 1.5 to 2 mm beyond the edge of the defect, completely closing the perforation. The cartilage transplant with a removed triangular section is then placed on the lower third of the malleus handle. The detached meatotympanic flap with the fascial flap fixed to the tympanic part of the tympanic membrane is returned to the place covering the cartilage transplant.

EFFECT: method allows for a high anatomical and functional result when closing perforations in the lower quadrants of the tympanic membrane and minimizes the postoperative rehabilitation period.
