FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to oncology, and is intended for plasty of a through defect of the oral cavity after block removal of the floor of the oral cavity, a fragment of the tongue, soft tissues and skin of the chin area in cancer patients. The method includes block removal of the floor of the oral cavity, a fragment of the tongue, soft tissues of the neck and skin of the chin area, while a free, chimeric, musculoskeletal flap from the fibers of the latissimus dorsi muscle and anterior serratus muscle is used as a plastic material on the thoraco-dorsal neurovascular bundle, then the vascular pedicle and nerves of the formed flap are crossed and transferred to the area of the defect, the distal end of the musculoskeletal part of the chimeric flap from the fibers of the latissimus dorsi muscle is fixed to the base of the epiglottis, and the proximal end is fixed to the frontal sections of the lower jaw, the remaining unfixed edges of the flap are distributed without tension and sutured along the perimeter to the body of the lower jaw and form microvascular anastomoses between the donor and recipient vessels and between the thoraco-dorsal, long thoracic nerves and the hypoglossal nerve, then the musculoskeletal part of the chimeric flap from the fibers of the serratus anterior muscle is placed in the projection of the floor of the oral cavity, the proximal end is fixed with musculoskeletal sutures to the body of the lower jaw, and the distal end is fixed with musculoskeletal sutures to the body of the hyoid bone, the skin-fat area above the anterior serratus the muscle restores the soft and integumentary tissues of the neck.
EFFECT: method allows to simultaneously eliminate a through defect of the oral cavity after block removal of the floor of the oral cavity, a fragment of the tongue, soft tissues and skin of the chin area and restore the shape and mobility of the remaining part of the tongue.
