FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used for the surgical treatment of children with plano-valgus deformity of the feet. The method includes preoperative patient preparation, tendon-muscle plasty, subtalar arthroeresis, osteotomy with autograft sampling from the fibula. An incision is made, dissecting soft tissues along the lateral surface of the foot, exposing the medial sphenoid bone. After determining the position, under visual control, the wires are inserted transversely to the medial surface of the bone into the proximal and distal fragments of the medial sphenoid bone. An osteotomy of the medial sphenoid bone is performed, a distractor in the form of a clamp with hinged jaws is used to dilute the bone fragments, one of which is equipped with an arcuate ruler, and the second with a "ratchet" type mechanism that allows you to displace and fix the handle of the second branch in the required place. Fixing pins located at opposite ends of the branches are installed in the distal and proximal bone fragments. The surgeon separates the fragments of the medial sphenoid bone in the osteotomy zone, thereby lowering 1 metatarsal bone by wedging the medial sphenoid bone by the required number of millimeters, marks the numerical indicator of millimeters on the ruler of the distractor, which should correspond to the thickness of the graft. At the same time, the required distance of dilution of bone fragments is fixed. Then, an autograft of the required and previously determined size is taken from the fibula of the same limb at the border of the middle and lower thirds. The resulting bone fragment is freed from soft tissues, a wedge-shaped autograft is formed from it and the size corresponds to the distance between the bone fragments. The graft is introduced into the zone of osteotomy of the medial sphenoid bone. The distractor is removed and the soft tissues are inspected and sutured, followed by the application of a plaster cast in the correction position for six weeks.
EFFECT: method provides sparing treatment due to accurate determination of the required number of millimeters of reduction of the 1st metatarsal bone and determination of the required graft size.
