FIELD: medicine; urology; gynecology.
SUBSTANCE: invention can be used to treat stress urinary incontinence. A method is proposed for the treatment of stress urinary incontinence with a volume-forming material as follows: the composition of the volume-forming substance includes collagen and autologous platelet-rich plasma, which are introduced into the paraurethral space, the method is carried out in several stages: preparation of platelet-rich plasma by double centrifugation, provided that the first centrifugation spin passes RCF 900 for 5 minutes, rotor radius is 18.8 cm, after the first centrifugation, the upper and middle layers are taken for the second spin carried out by RCF 1500 for 15 minutes, rotor radius is 18.8 cm, after which the upper 2/3 of the resulting drug is removed, and the remaining 1/3 is platelet-rich plasma; carrying out laboratory control of the obtained plasma and upon receipt of the platelet count 3–5 times higher than the baseline, the drug is activated with calcium chloride 1:10; conducting application anesthesia using a 5 ml gel containing lidocaine and chlorhexidine, with an exposure of 10 minutes; conducting local anesthesia with a solution of Ropivacaine 7.5 mg/ml, prepared by diluting up to 2 ml of NaCl 0.9% in a ratio of 1:1; preparing a volume-forming substance by mixing collagen and platelet-rich plasma in a ratio of 2:1, respectively; performing paraurethral injections with a needle for spinal anesthesia of the Quincke type, 20G thick, 70–90 mm long, depending on the length of the urethra, injections are performed along the urethra, parallel to it, in the paraurethral region at the level of the middle and proximal thirds of the urethra at 4 points on 2, 4, 8, 10 hours of the conditional dial of 1–3 ml under direct endoscopic control, controlling the degree of coaptation of the urethral mucosa; follow-up of the patient for 1–3 hours to assess the recovery of urination and assess the residual volume of urine, with dynamic control at 1, 4, 8 months after treatment.
EFFECT: above method allows to increase the resistance of the urethra, increase the degree of coaptation of the urethral mucosa due to compression with a volume-forming substance, which allows leveling the clinical symptoms of the stress form of urinary incontinence, improving the patient's quality of life.
