METHOD OF PLASTY OF POST-TRAUMATIC PERIARTICULAR DEFECTS OF LONG TUBULAR BONES (OPTIONS) Russian patent published in 2023 - IPC A61B17/56 

FIELD: medicine, traumatology and orthopedics.

SUBSTANCE: revision of bone fragments is carried out with the formation of a bone bed and free bone fragments. The periarticular defect is repaired with a graft. After implantation of the latter into the bone bed, revascularizing osteoperforation is performed by forming parallel transosseous channels along the longitudinal axis of the bone diaphysis, passing through the graft and communicating with the bone marrow cavity. Then, free bone fragments are placed on the graft and pressed, thereby ensuring the compaction of its spongy substance. Fixation of the graft is carried out by means of metal osteosynthesis with extraosseous plates and screws with angular stability passed through them in mutually intersecting planes. A distinctive feature of the second embodiment of the method is an additional element for fixing the graft in the bone bed made by processing the latter with the formation of a groove and modeling a protrusion in response to it on the surface of the graft in the zone of their contact.

EFFECT: use of the claimed group of inventions makes it possible to create conditions for potential osseointegration of the bone bed with the graft due to the accumulation of the effective volume of the cell mass prolonged in time, and the subsequent consolidation of the fracture, as well as to ensure stable osteosynthesis due to the multiplanar fixation of the graft in the bone bed.
