FIELD: medicine; ophthalmology.

SUBSTANCE: invention can be used for combined conservative treatment of active endocrine ophthalmopathy. From the first to the 13–17th day of treatment, methylprednisolone is administered intravenously at an initial dose of 10–12 mg/kg of the patient’s body weight, diluted in 0.9% saline solution, in the first half of the day, for 35–45 minutes, after day, decreasing or increasing the previous dose by 250 mg/day through 1–2 droppers, depending on the dynamics of signs of inflammation in the eye and orbit, up to 4 mg/kg of the patient’s body weight. A total of 7–10 droppers are required for a course. From the first day of treatment, for 14 days, the patient takes omeprazole 20 mg orally daily, 1 tablet 2 times a day. Also, the patient is injected intramuscularly with a solution of deproteinized calf blood hemoderivate 2.0 ml and a solution of methylethylpyridinol at a dose of 1.0 ml once a day. Installations of a solution of dexamethasone 0.1%, 1 drop 4 times a day, and a solution of methylethylpyridinol 1%, 1 drop 4 times a day are performed. From the first day of treatment, a parabulbar injection of betamethasone solution is performed at a dose of 1.0 ml. 1 injection is performed with an interval of 4–6 days, for a total of 3 injections for each eye per a course. From the first day of treatment, the patient is given magnetic therapy for 10–12 days in the area of the affected orbit using ophthalmic emitters of the Polimag-02 device, which are contact-installed in the projection of the affected orbit. Impact parameters are as follows: mode is continuous, type of magnetic field is clockwise rotation, induction is 4–6 mT, frequency is 2–5 Hz, exposure time is 15–20 minutes. From the second day of treatment and further every other day until the 14th day, a solution of furosemide 1% 2.0 ml is administered intramuscularly. Also, from the second to the 14th day of treatment, the patient administers 1 tablet of potassium aspartate and magnesium aspartate daily orally 3 times a day; the course of treatment is repeated. In case of development of complications of endocrine ophthalmopathy, such as non-closure of the palpebral fissure, erosion or corneal ulcer, additional installations of levofloxacin 0.5%, 1 drop 4 times a day, are carried out from the 1st to the 7th day of treatment. Also, installations of bromfenac 0.09%, 1 drop 1 time per day and dexapanthenol 5%, 1 drop 3 times a day from the 1st to the 14th day of treatment are performed.

EFFECT: invention ensures the achievement of a rapid, pronounced and long-lasting therapeutic effect, improvement of the patient’s general condition, reduction of the risk of vision loss, increase in the duration of remission of the process, improvement of the patient’s cosmetic and psychological condition.
