METHOD OF SURGICAL TREATMENT OF POSTHYSTERECTOMY PROLAPSE Russian patent published in 2023 - IPC A61B17/42 A61F2/08 

FIELD: medicine; coloproctology.

SUBSTANCE: anterior and posterior colpotomy is performed as follows: excision of the anterior and posterior vaginal wall, isolation of the levator ani muscles, posterior colporrhaphy and sphincterolevatoplasty are performed. The enterocele is isolated, with dissection of the hernial sac, the contents of the enterocele are immersed in the abdominal cavity, the hernial sac is excised, the remains of the hernial sac are sutured with a purse-string suture, and a free end of the thread is left. Then a mesh prosthesis of 4.0 cm by 3.0 cm is fixed with two interrupted sutures subserosally to the posterior surface of the cervical stump or to the dome of the vagina. The prosthesis is positioned so that the center of the prosthesis is located on the remains of the hernial sac, previously sutured with a purse-string suture. The previously left free end of the thread is used to fix the mesh prosthesis to the remains of the hernial sac using an interrupted suture. Also, the mesh prosthesis is fixed in the area of the upper third of the vagina with two interrupted sutures to the pubococcygeus muscle, after which the stumps of the uterosacral ligaments are sutured over the mesh prosthesis.

EFFECT: method makes it possible to increase the effectiveness of surgical treatment by reducing the number of relapses of pelvic organ prolapse, ensuring the physiological position of the vagina, and correcting dysfunction of the bladder and rectum.
