FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgical gynaecology. Intrafascial hysterectomy involves mobilization of uterosacral ligaments to create a safe distance from the ureters. After the uterus is dissected away from the fornix, a vaginal vault is closed with a continuous two-row suture with covering proximal portions of pubic-cervical, rectovaginal fascias and uterosacral ligaments. Further, the peritoneum is exposed in the lateral and anterior direction from a junction of the medial umbilical folds and round ligaments of the uterus; inner surfaces of the pubic bones with pectineal ligaments to which a vaginal vault is symmetrically fixed by two separate interrupted sutures with Etibond No.0 tensioned sutures are exposed. Uterosacral ligament is plicated from both sides with separate U-sutures with Etibond 2/0 suture with Douglas pouch obliteration. Surgery is completed by peritonisation of the wound canals within the Retzius space.

EFFECT: method allows to perform single-step correction of apical prolapse of pelvic organs during laparoscopic and robot-assisted hysterectomy, prevent recurrent omission of both anterior-apical, and posterior vaginal compartments, avoid implant-associated complications in the postoperative period and reduce the cost of the intervention by avoiding the use of polypropylene endoprosthesis.
