FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to otorhinolaryngology. Two-staged surgical intervention is performed on the middle ear. Covering element is formed as a gasket from polymer material. Gasket is placed at the first stage, and not earlier than 6 months later at the second stage it is removed. Gasket is made with dimensions: 0.2 mm thick, 11.0 mm long, 6.9 mm wide. Shape of the gasket is obtained on the basis of the following anatomical measurements: the distance between the oval and round window; longitudinal and transverse dimensions of the oval window; distance between round window and spoon-shaped appendix; distance between the front edge of the oval window and the spoon-shaped process; width of the tympanic fistula of the auditory tube in a projection of the prototype; distance between the round window and the most distant point of the canal of the muscle stretching the tympanic membrane; distance between a posterior wall of the tympanic cavity in the tympanic sinus and a point of the bone tympanic ring corresponding to a projection of the middle of the tympanic mouth of the auditory tube. If the superstructures of the stapes are preserved, the gasket is cut out in accordance with Fig. 1. In this case, the gasket covers the tympanic mouth of the auditory tube, the above-tubular space, the promontory of the tympanic cavity, spoon-shaped process, section of medial wall of tympanic cavity between niche of oval window and spoon-shaped process, niche of round window, sinuses of retrotympanum. In case of loss of stapes superstructures, the gasket is cut out in accordance with Fig. 2. In this case, the gasket also covers the recess of the oval window. Then, not earlier than in 6 months, the second stage of surgical intervention is performed. Gasket is removed from the tympanic cavity, and the ossicular chain is reconstructed.
EFFECT: method enables improving the long-term functional results of tympanoplasty by preventing the development of a cicatrical-adhesive process in the tympanic cavity due to the barrier between the inner and outer walls of the tympanic cavity.
