SUBSTANCE: invention relates to medicine, particularly to otorhinolaryngology. At the first stage, the operation involves a postaural incision, separation of the meatotympanic flap, inspection of the tympanic cavity, sanitation thereof, removal of tympanosclerosis foci, assessing the state of auditory ossicles, myringoplasty by laying of fascial autograft. Fixed anvil and the head of the hammer are additionally removed before the myringoplasty. Rotating the handle of the hammer backwards, comparing it with the head of the immovable stirrup, imitating in this position a long process of the anvil above the window opening and at a distance corresponding to the height of the stirrup superstructures. Then the silicone tread is placed on the medial wall of the tympanic cavity and the fascial autograft is brought down to full contact with the hammer handle. At the second stage of the surgery, the fixed stapes superstructures are removed. Performing stapedotomy, installation of a prosthetic prosthesis with its fixation on the handle of the hammer. At each of the stages, the separated tissues are placed in place and the wound is closed. Silicone tread is removed, and the prosthetic prosthesis is placed by immersing its proximal part through perforation in the perilymphatic space. Distal portion of the prosthesis-piston is fixed on the handle of the hammer.

EFFECT: method enables improving the efficiency of the operation by improving sound transmission and simplifying the procedure for selecting the length of a piston-type prosthesis.
