FIELD: medicine.

SUBSTANCE: invention relates to reconstructive and maxillofacial surgery. Entire thickness of the affected part of the lower eyelid and the medial commissure of the eyelids are excised to the bone, including its tarsal part and a portion of the conjunctiva. Then from skin of nasolabial fold of the same side is cut out transposition skin and fat flap, as shown in Fig. 1, 5/4 of the size of the excised eyelid. Flap tip is located at the lip level, and the flap base is located inside and above the medial angle of eyes by 0.5–1 cm. That is followed by a sharp de-epithelization of the ciliary side of the edge of flap 5 mm wide from the base to the tip. Then cutting off the upper eyelid stump from the lateral commissure from the conjunctiva, upper eyelid stump is displaced medially and sutured medially to the base of the flap, thus forming a medial commissure of the eyelids; the tip of the flap is sutured end-to-end to the lateral commissure of the eyelids. De-epithelial edge of the flap is used to form a ciliary edge of the reconstructed lower eyelid. Further, the donor wound is closed along the nasolabial fold with Prolen 5–0 suture, while preserving the pedicle within the medial angle of the eye; skin sutures are removed on 7–10th day.

EFFECT: method enables increasing the efficiency of the plastic surgery due to its single-stage nature and the possibility of reconstructing the entire thickness of the lower eyelid.
