FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to plastic and aesthetic surgery. Endoprosthesis is approached by skin incision, tissue dissection, endoprosthesis capsule dissection and endoprosthesis extraction. Position of attachment of the fibres of the greater pectoral muscle to the sternum is assessed. In the case when the greater pectoral muscle is cut off from the sternum above the projection of three o’clock on the right or nine o’clock on the left, on the clock face, as shown in Fig. 3, a lower edge of a muscular stump of a greater pectoral muscle is mobilized. Then, in the projection of the submammary fold, the proper thoracic fascia is approached with electric knife and the caudal dissection is suprafascially performed, forming a submuscular pocket, along borders: medially – parasternal line, laterally – nipple line, cranial – projection of submammary fold, caudal – distance equal to distance from projection of submammary fold to edge of greater pectoral muscle. Fascial flap is then separated along the medial, lateral and caudal borders, and the cranial border is the base of the flap and its pedicle, then edges of the formed fascial flap and greater pectoral muscle are sutured with single interrupted sutures, forming a "hammock". Further, 50 units of botulinum toxin type A diluted with 5 ml of normal saline under visual control are uniformly injected into a greater pectoral muscle, new implant is placed in the formed submuscular pocket and the postoperative wound is closed in layers.

EFFECT: method enables providing the correct positioning of the breast implants during repeated operations on the mammary glands to achieve the maximum aesthetic and functional result.
