SUBSTANCE: invention refers to medicine, namely to plastic surgery. Method includes preoperative marking of anatomical formations, hydropreparation of mammary gland and greater pectoral muscle, formation of access, dissection of tissues, formation of bed for implant and its placement, layer-by-layer wound closure. An approach is formed by performing a skin incision in a submammary fold from a vertical nipple line projected onto the submammary fold in the direction of anteroaxillary line 4.5–5.5 cm long. A bed for the implant is formed by dissecting breast soft tissues laterally from a diagonally directed segment – segment V located in a layer between leaves of a pectoral fascia muscle and a posterior leaf of a superficial breast fascia within a preoperative marking. Then, a fascia of the greater pectoral muscle is incised at level of 5th intercostal space with length of 1–2 cm on a segment V; then, through the transfascial incision, the fibres of the greater pectoral muscle are bluntly separated to the level of the retromuscular space. Further, the lateral portion of the inferolateral portion of the greater pectoral muscle is used to form a lateral wall of the bed, and the formed V-shaped two-flap bed consisting of the superior medial and inferolateral portions of the greater pectoral muscle is drained and an implant is inserted.

EFFECT: method provides the reliable and stable positioning of the breast implants on the breast wall, prolongs the mammoplasty result, and also eliminates the probability of the implant dropping below the inframammary fold.
