SUBSTANCE: invention refers to medicine, namely to cardiovascular surgery, and is intended for combined treatment of patients with critical ischemia of lower extremities with underlying diabetes mellitus. After the conservative treatment and switching the patient from taking the tablet preparations for diabetes mellitus to insulin, a femoral-foot shunting is performed with a reversed autovein or a combined conduit. In the postoperative period, the patient is administered Vazaprostan two or three ampoules, Octolipen 600 mg for 10 days, then Octolipen 600 mg for 90 days; Vessel Due F 600 LSU intravenously drop-by-drop for 10 days, Vessel Due F 250 LSU tablets up to 60 days. For the whole period of postoperative inpatient and outpatient treatment, the patient undergoes anticoagulant therapy to prevent shunt thrombosis, Eliquis 2.5 mg and Thrombo ASS 100 mg are prescribed; non-steroidal anti-inflammatory drugs are administered. Under the control of endocrinology at the stage of hospital treatment, daily glucometry is performed with correction of glucose level by subcutaneous administration of insulin; Actovegin 10.0 ml is introduced intravenously drop-by-drop for 10 days, then Actovegin 5.0 ml is introduced intramuscularly for 10 days, then Actovegin in capsules is introduced for 120 days; Neuromultivit is introduced intramuscularly for 10 days, then Neuromultivit in tablets for 30 days. EFFECT: invention allows to normalize vascular wall endothelial functions, to stimulate angiogenesis on working shunt in case of shin artery occlusion.
