FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used in reconstruction of osteochondral defects of joints of extremities. Method involves drilling pathological bone tissue to the depth of the necrosis zone with the formation of cylindrical bone defects with a hollow cylindrical cutter, with preservation of bridges between them, sampling of autologous cylindrical bone grafts from lateral periarticular parts of limb bones, their implantation into formed cylindrical defects, modelling the collagen membrane by the shape and size of the defect, covering the defect area with it, with fixation to the surrounding hyaline cartilage, filling the donor holes in the condyles with allograft. To obtain a spongy cylindrical graft, a cortical layer is removed from the donor site. For this purpose, the cutter is immersed to the depth of the cortical layer, then it is removed and cleaned from the cortical layer of the bone. Same cutter is used to collect decorticated bone columns of the required height and diameter. After the collagen membrane is fixed, the joint is moved.
EFFECT: method provides the creation of optimal conditions for the migration of bone marrow cells to the articular surface for the formation of hyaline-like cartilage, which enables to prevent the development of osteoarthrosis by levelling the thickness of the cortical layer, before sampling and introducing the graft, allows not destroying the graft and performing implantation directly from the cylindrical cutter, and the use of the collagen membrane enables to retain the emerging growth cells and growth factors on the surface in the defect area, and also protects against the action of tangential forces, when performing joint movements during rehabilitation treatment, until membrane resorption.
