FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to orthopaedics, and can be used for surgical treatment of hallux valgus. Method involves approach along the inner surface of the foot to the distal one-third of the first metatarsal bone and the base of the proximal phalanx of the first toe, drawing the articular cartilage line on the back surface of the head of the first metatarsal bone between its lateral and medial extreme points. Distal and proximal transverse cuts of the first metatarsal bone are marked. From the most distal point of the line of the longitudinal axis of the first metatarsal bone, a line is drawn parallel to the line of the longitudinal axis of the proximal phalanx of the first finger to the line of the distal transverse saw cut, and a point is marked on it. First metatarsal bone is cut longitudinally, distal and proximal, to form a plantar and back fragments, and the plantar fragment is brought into the wound. Two lines are drawn on the plantar fragment from a point on the line of the distal cross-cut: the first line is parallel to the line of the articular cartilage to the medial edge of the bone, the second line is drawn in the proximal direction perpendicular to the line of the articular cartilage to the edge of the bone. Fragment obtained during marking is cut out by performing three cuts: the first one is cut along the first line to the depth to the surface of the longitudinal sawing of the sole fragment plus 2 mm, the second cut along the second line to depth of 2 mm, and the third cut from the medial edge of the fragment to the first and second cuts, as a result of which an angular recess with two sides is formed. Thereafter, the plantar fragment is displaced laterally, the formed angular recess is brought behind the distal lateral angle of the dorsal fragment, the fragments are tightly pressed together and fixed with a screw.
EFFECT: method provides a more accurate correction of the deformation of the first toe by performing the lateral displacement of the plantar fragment of the first metatarsal bone by an individual value taking into account the degree of hallux valgus, achieving optimal distribution of load on heads of metatarsal bones, prevention of recurrent deformation of the first toe, development of transient metatarsalgia and deformation of the second toe and restoration of statodynamic function of foot.
