SUBSTANCE: invention refers to medicine, namely to ophthalmology, and can be used for surgical treatment of primary through macular retinal rupture. For this purpose, an upper inverted single-layer flap of the ILM is formed, which is turned over and the inner side is laid on a macular rupture, MR. In other areas of the retina, the ILM is not removed. Further, fluid is replaced with air by directing air supply to the centre of the macular area of the retina, and the opening of the vitreous cutter is placed under the free edge of the single-layer flap of the ILM, outside the inferior temporal vascular arcade, outside the macular area of the retina. Flap of the ILM is dried until the surface of the ILM flap is glazed in the medium of air and is static. Thereafter, 5 drops of autologous conditioned blood plasma ACP are applied on the surface of the dried inverted flap of the ILM and exposed for 5 minutes. Then, air is replaced with BSS using 20 ml syringe and 27G curved cannula. Operation is completed without tamponade of the vitreal cavity.

EFFECT: invention provides reduced number of intraoperative manipulations, minimizes risk of intra- and postoperative complications, closure of macular retinal rupture with restoration of normal histological architecture of retina in fovea, high functional result of treatment.
