FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to ophthalmology. Surgical treatment of macular rupture is performed, in which at the first stage a transconjunctival three-port vitrectomy is performed through a flat part of the ciliary body, balanced salt solution (BSS) is supplied through an irrigation system into a vitreal cavity. Then, posterior hyaloid membrane and adjacent posterior layers of vitreous body are removed, thereafter, an epiretinal membrane and an internal boundary membrane are removed after preliminary staining with an intravitreal dye around the foveola within the vascular arcades. At the second stage, the BSS is replaced with a perfluoro-organic compound (PFOC) in amount of 1/4 to 1/3 of the volume of the vitreal cavity; the pressure of the irrigation fluid is set within range of 10 to 15 mmHg. An aspiration system with an extrusion needle is introduced into the vitreous cavity and filled with an irrigation liquid, in passive aspiration mode, an extrusion needle tip is passed through the PFOC medium close to the macular rupture zone, without touching the retina, performing passive aspiration, as a result of which the defect of the torn retinal tissue is closed, the degree of closure of the edges of the macular rupture is visually controlled. Exposure of the introduced volume of PFOC on the area of the closed macular rupture is not less than 10 minutes; during this time, the patient’s blood is sampled and autologous conditioned plasma (ACP) is prepared. After receiving the ACP prepared for the introduction by the surgeon, the PFOC is aspirated, and in the medium of the irrigation liquid, the ACP is applied on the area of the closed edges of the macular rupture, volume of one layer is up to 0.05 ml, the number of layers is 2–4, one layer is applied on top of another with exposure of up to 1 min in volume of 0.1–0.2 ml until a film appears within the vascular arcades. Operation is completed by exchanging the previously introduced BSS for air; the sclerostomas are sutured.
EFFECT: method enables to achieve elimination of metamorphoses, improve and stabilize visual functions, light sensitivity of retina, restore the macular retinal architectonics with formation of the correct foveolar profile, reduce the defect repair area.
