FIELD: medicine; neurosurgery. SUBSTANCE: access to vertebral artery is provided over rear edge of upper third of sternocleidomastoid muscle with dissection out of outer edge of bandage of the neck at occipital bone, cutting of tendon of inferior oblique muscle of the head, musculus semispinalis and superior oblique muscle at point of attachment to transverse process of first cranial vertebra, decompression of second spinal ganglion and second neck nerve. Decompression and removal of artery fixation in holes of transverse processes of cranial and second cervical vertebras are effected by resection of hole rear wall with the aid of ultrasonic saw. Mobilization of artery at the level of atlantoaxial and atlantooccipital joints by section of adhesions and processes between artery adventitia and joint capsule. Operation is ended in bringing of fatty autotransplant between capsule of atlantoaxial joint, wall of vertebral artery and front branch of second neck nerve bending vertebral artery round its outer and rear sides, implantation subepineural of microelectrode for electrostimulation in postoperative period into rear branch of second neck nerve. EFFECT: higher efficiency of management of compression occurred at the level of third segment. 2 cl
