One piston is rigidly connected to the rod. The second piston is also rigidly connected to the rod on the opposite side from the lubricated threaded joint. The cavity under the second piston is isolated from its over-the-piston cavity which is continuously in communication with the atmosphere. The piston diameters are equal.
