SUBSTANCE: the present innovation deals with median laparotomy and mobilization of pancreatic caput by certain technique being differentiated by the fact that one should longitudinally dissect the capsule and surface pancreatic parenchymal layer along anterior surface of the caput, intraparenchymatously remove the tissue mass from pancreatic caput by keeping parenchymatous-capsular carcass of 0.5-0.7 cm thickness, dissect anterior wall of pancreatic duct opening into the cavity developed followed by isolation of small intestinal section to be then anastomosed with parenchymatous-capsular carcass that restricts the pancreatic caput's cavity, and with a pancreatic duct that keeps organ's vessels that enables to carry out optimal drainage of pancreatic ductal system and excludes squeezing of intrapancreatic part of choledochus. EFFECT: higher efficiency of surgical treatment. 1 ex
