FIELD: medicine.

SUBSTANCE: isolated subtotal pancreatic head resection and pancreatoenterostomy are performed. A pancreatojejunoanastomosis is created. The common bile duct is exposed. A stent is placed into a cavity formed after the pancreatic head resection. For placing the stent, a silicone tube is put on a head of a metal guide with the stent placed at the opposite end of the tube. The stent is made by plaiting a flat-section TN-10 titanium nickelide wire. The wire width is from 0.8 to 2 mm; its thickness is 0.3-0.8 mm; a mesh size is 1-2 mm. The stent diameter is equal to that of the duct. The metal wire is taken out by reverse traction, and the stent is placed into the common bile duct towards the portal fissure. The stent is fixed by two stitches to the pancreatic parenchyma. The choledochotomy opening is either closed tightly, or drained externally temporarily. The stent is additionally placed into a lumen of the distal portion of the pancreatic duct.

EFFECT: method enables eliminating complications of chronic pancreatitis, duodenum and common bile duct stenoses, compression of retropanreatic and great intestinal vessels effectively, preventing pyoinflammatory postoperative complications by using the nickelide titanium stent having optimum structural characteristics, and the absence of the necessity to form an anastomosis of an intrapancreatic portion of the common bile duct, ensures non-traumatic stent delivery by using the metal guide with the silicone tube.
