FIELD: medicine, surgery.

SUBSTANCE: the present innovation deals with operative treatment of patients with complete blockade of distal department of common biliary duct and cholecystostoma applied earlier. One should perform pre-operational doppler output-cholangiomanometry. Due to droplet introduction of warm sterile physiological solution of common salt one should create the pressure in biliary ducts being equal to 300 mm water column, open outflow tube, perform decompression of biliary ducts, detect volumetric rate of liquid outflow along cystic duct. Trial should be repeated at filling biliary ducts up to 400 mm water column pressure. At volumetric liquid flow through cystic duct being above 0.5 ml/sec at initial pressure being 300 mm water column and above 2 ml/sec at initial pressure being 400 mm water column one should conclude upon sufficient permeability of cystic duct and possibility to apply patient's gallbladder for applying bilio-digestive anastomosis. The method enables to detect functional permeability of cystic duct in patients with complete blockade of distal department of cystic duct and cholecystostoma applied earlier with noninvasive technique and match the most favorable type of bilio-digestive anastomosis for each patient. It, also, decreases the chance for the development of purulent-septic complications.
