SUBSTANCE: a questionnaire should be suggested for a patient that consists of three fragments. Moreover, the first fragment evaluates the function of the upper limbs and deals with evaluating the difficulties in case of doing up one's shoe-laces, buttoning up, taking a filled glass or a cup to drink, washing one's head; the second fragment evaluates the function of the lower limbs and deals with evaluating difficulties at walking outside along even surface, stepping upstairs for 5 steps, sitting and standing up from a toilet bowl; the third fragment evaluated the function of vertebral column and deals with evaluating patient's abilities to lie down and stand up from the bed, take bath, bend to take something fallen to the floor (clothes). Moreover, it is necessary to evaluate the rating of predominance of this or that fragment by visual analog scale (VAS) by taking into consideration pain intensity and making at the scale the site that corresponds to patient's ability to fulfill the action (according to patient's opinion). Moreover, "0" corresponds to unaffected implementation of the action, and "100 mm" - to absolute inability to fulfill any action, the evaluations obtained in each fragment should be summarized to be divided by 3, and curative physical training should be prescribed for this or that department of motor apparatus, the fragment's rating of which predominates. The innovation is efficient, more simplified and available in implementation.
